Many people prefer taking a more self-help approach to working with psychological problems such as anxiety and depression, and that is a very good movement. Typically, you will gain much more benefit if you see yourself as being in charge of the process of change rather than going to a therapist and taking on the role of a patient, for example. That tends to put you in a weaker position and can actually get in the way of change.

I prefer encouraging a much more empowered approach in which you are directing your own process of transformation and healing.

The Mindfulness Therapy approach that I offer during these online Skype therapy sessions is particularly effective for working with anxiety and depression in this way.

The focus is really on teaching you two very essential skills. The first is learning how to change the nature of your relationship to your emotions; and the second approach we teach during these mindfulness therapy sessions is learning how to investigate and change the actual structure of the emotion, the actual process that operates habitually to create anxiety or depression.

So, in the first approach, the first part of mindfulness therapy, it is all about establishing the right kind of relationship with your emotions.

We have to break the habit of avoidance and resistance, or aversion. Avoidance and aversion are your worst enemies. These prevent anxiety or depression from changing and healing. We have to actually learn to do the opposite to avoidance and aversion.

We have to learn to embrace our emotions in full consciousness and with a quality of friendship. We need to learn to see our emotions as being parts of our self that are in pain and require our help; they require you to develop a relationship based on compassion with your anxiety or depression or other emotions.

This is what promotes change and with all of the clients that I have worked with over the years, I have found this to be most essential predictor of change; and that is when you can begin to establish a friendly relationship with your anxiety or depression or painful emotions, without becoming overwhelmed and without becoming reactive, then the emotions will change very substantially. They need this quality of relationship in order to heal.

So, you have to learn how to become a friend to your emotions. Learn to see the emotions as being like visitors, as being parts of you that are in pain and that need your conscious and compassionate help in order to change.

The second part of the mindfulness therapy approach that works so well for psychotherapy online, is learning how to change the structure of emotions.

All emotions have a structure, and that structure is based on imagery; how you see the emotion or the thoughts in the mind is what actually determines the intensity of the emotion. This is a vital factor that we investigate during the therapy sessions together. We start to actually investigate how your emotions work and what kind of imagery is creating those emotions.

The more you learn about the imagery of the emotion the more you can change that imagery, and when you change that imagery, you change the emotion. So, this is quite an essential part of the transformation and healing process is working with emotional imagery.

Is Online Psychotherapy as Effective as in-person Therapy?

There is a growing body of evidence to support the claim that online psychotherapy is a good quality alternative to traditional “office-based therapy.” Of course, the comparison depends on what form of psychotherapy is being compared. Generally, the evidence supports the use of Cognitive Therapy (CBT). A controlled study conducted at the University of New South Wales, Australia, found that internet therapy over an 8-week period produced significant improvements for subjects suffering from depression when compared to a control group. In fact, 34% of subjects “no longer met the diagnostic criteria for depression.” That is actually quite a big change after such a short course of psychotherapy.

In my experience with Skype-based psychotherapy, I would say emphatically yes to that question. One of the advantages of online psychotherapy is that clients feel much more comfortable and less intimidated when working from the comfort of their own home – and feeling comfortable is a very important factor for successful psychotherapy.

Can I continue taking my anxiety medication during Online Psychotherapy?

It is generally agreed that the most successful treatment for anxiety and panic attacks involves a combination (Combination Therapy) of anti-anxiety medications, prescribed by your medical doctor, and some form of psychotherapy. Medications treat the symptoms, but psychotherapy is needed to change the underlying cause of your anxiety disorder or depression. Often, people will use antidepressants to reduce the severity of their symptoms to a manageable level and then seek psychotherapy to work on the underlying emotions that need to be resolved and healed. Hopefully, after a course of therapy you will be able to stop taking medications. Most people do not like taking medicines and would prefer to stop when they feel ready.


How many sessions of psychotherapy will I need?

Of course, this will vary from person to person, but I feel strongly that the client should see noticeable changes after each session. It should not require years of psychotherapy to get better. In the Mindfulness Therapy approach, which is very focused and relies on teaching you practical exercises to practice between sessions, most people experience major changes in their anxiety or depression after 4-6 sessions.

    • This is true, as is the case for conventional therapists.If you are considering a therapist, online or in-person, make sure that they answer all your questions, that you feel comfortable with them personally, and that you believe in the therapeutic approach that they provide. Also, understand that qualifications alone do not signify that a therapist is the right choice for you, this is why you must ask the therapist to explain his or her approach and that you feel that their approach fits with your beliefs. Also, a good therapist should be able to provide you with a plan of action with a time-line of when you should expect to see and feel results. Just talking endlessly about your emotions does not constitute good psychotherapy in my opinion. Much more important is to address and resolve the underlying causes and processes that create your emotional problems.

    • However, unqualified psychotherapists can also have an office as well. The medium makes no difference. There are plenty of unqualified professionals in every field in every possible location. The virtue of being online doesn’t change the required due diligence. How does someone know that when you go to a therapist’s physical office that they are legitimate? The simple way is to be sure the therapist has a valid license. Check the license number with the licensing authority website.

      • Good advice. Of course there is an assumption that licensing equates with professional competence, and I am not sure that is entirely true. My advice is 1) never agree to pay for a therapy session up-front, only after satisfactory completion of the session, and 2) ask your questions about what to expect from sessions, the therapist’s approach and philosophy, and other points like this. You will soon be able to tell if the individual is skilled in his art and worth trying. To be a qualified psychotherapist means that you are effective in helping people change and that rarely if ever equates to academic degrees or certifications in psychology.

      • Good points. You should make sure you can correspond or interview a therapist before you pay for sessions. Even licensing does not in any way guarantee that the therapist is competent or that his approach is right for you. Trust your intuition and the results you experience. My policy is “try first, pay after if satisfied.”
